AMSI has collected the following categories of personal information from consumers within the past twelve (12) months:
- health insurance information;
- personal identifying information, like name, alias, address and account name and number, financial information, social security number, as well as other identifying information;
- characteristics such as age, gender, etc.;
- retail information;
- audio, electronic, or similar information including call recordings. Recordings are made when the consumer has a telephone conversation with us. (NOTE: calls made by AMSI to consumers and calls made by consumers to AMSI are recorded and may be monitored);
- Commercial information, including records of personal property, products or services purchased;
- professional and employment related information;
- Inferences drawn from other information.
AMSI collects most of this personal information from our clients or from you or your authorized representatives by telephone or by electronic or other written communications. We may also collect information about you from our service providers, from third parties and from third parties with your consent.
AMSI does not sell personal information to third parties.
Data processing and storage
The personal information we collect is stored and processed in the United States, or where we or our partners, affiliates and third-party providers maintain facilities.
We only retain personal information for as long as necessary to provide a service, or to improve our services in the future.
Third-party access to information
We may, from time to time, allow limited access to our data by external consultants and agencies for the purpose of analysis and service improvement. This access is only permitted for as long as necessary to perform a specific function. We only work with external agencies whose privacy policies align with ours.
We may refuse government and law enforcement requests for data if we believe a request is too broad or unrelated to its stated purpose. However, we may cooperate if we believe the requested information is necessary and appropriate to comply with legal process, to protect our own rights and property, to protect the safety of the public and any person, to prevent a crime, or to prevent what we reasonably believe to be illegal, legally actionable, or unethical activity.

Under the California Consumer Privacy Act (CCPA), California residents may have the right to request, free of charge, that a business disclose certain information. Once we receive and confirm your verifiable consumer request, we will disclose:
The categories of personal information that it has collected about that consumer.
The categories of sources from which the personal information is collected.
The business or commercial purpose for collecting or selling personal information.
The categories of third parties with whom the business shares personal information.
The specific pieces of personal information it has collected about that consumer.
To send us a verifiable consumer request for information, and the specific information you request, include your full name, address, date of birth, account number if applicable, and the last 4 numbers of your Social Security number by calling toll free: (800) 922-4704, or by contacting the information to: firstname.lastname@example.org.
A consumer can designate an authorized agent to make a request under the CCPA by sending us a notarized power of attorney. You must verify your identity and provide authorization to speak with your authorized agent. You must describe your request with sufficient detail to enable us to respond to your request.
You have the right to request that we delete your personal information, subject to certain exceptions. If the information AMSI maintains about you falls into one or more exceptions, AMSI may not be able to delete the information.
A consumer has a right not to receive discriminatory treatment by us for the exercise of the privacy rights as described herin.
